CHARTER OF RIGHTS FOR PEOPLE LIVING WITH A DEMENTIA.
I have the right to be provided with accessible information and support in order to participate in decisions which affect me.
I have the right to full participation in my care needs assessments including advanced decision making.
I have the right to live as independently as possible with access to recreational,leisure and cultural life in my community.
I have the right to be treated as an individual,with dignity,respect and privacy at all times.
I have the right to be free from discrimination on any grounds including age,disability,gender,race,social status,sexual orientation and religious beliefs.
I have the right to health and social care services provided by professionals and staff who have had appropriate training on dementia and human rights.
I have the right to be kept safe from harm and abuse and protected legally should my capacity diminish,free from restraint physically and chemically.
Above all,I have the right to be me!
